POSITION SUMMARY

The hospice aide is a member of the interdisciplinary group who works under supervision to provide personal hygiene care to hospice patients, following the patient’s Plan of Care.


ESSENTIAL FUNCTIONS

  • Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
  • Meet safety needs of the patient using equipment safely and properly (foot stools, side rails, O2, etc.)
  • Deliver routine medical supplies as needed to patients.
  • Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
  • Observe and report any patient condition changes to the RN Case Manager.
  • Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
  • Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT for providing excellent customer service.
  • Successfully complete 12 hours of in-service training annually.
  • Assist the patient with transfers as needed.
  • Exhibit respect of patient and family/caregiver environment and patient’s personal needs.
  • Provide compassionate companionship to patients while delivering services.

 POSITION QUALIFICATIONS 

Requires a HS degree or GED, and proof of State Certification

 1 year of direct patient care experience required
