POSITION SUMMARY
The hospice aide is a member of the interdisciplinary group who works under supervision to provide personal hygiene care to hospice patients, following the patient’s Plan of Care.
ESSENTIAL FUNCTIONS
- Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
- Meet safety needs of the patient using equipment safely and properly (foot stools, side rails, O2, etc.)
- Deliver routine medical supplies as needed to patients.
- Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
- Observe and report any patient condition changes to the RN Case Manager.
- Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
- Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT for providing excellent customer service.
- Successfully complete 12 hours of in-service training annually.
- Assist the patient with transfers as needed.
- Exhibit respect of patient and family/caregiver environment and patient’s personal needs.
- Provide compassionate companionship to patients while delivering services.
POSITION QUALIFICATIONS
Requires a HS degree or GED, and proof of State Certification
1 year of direct patient care experience requiredImages
